[review] => Was looking for a care facility for my Mom when she required too much care for me and my family to provide. Went to Boynton Beach Rehab for an unannounced tour and was fortunate enough to have the Head Administrator, John Kelly, give my wife and I a tour. John was great, as were Mary and Martin in Admissions. The facility has very nice outdoor space and the food is above average. Jean and Ce Ce are excellent CNA's, Celeste is a great nurse and the PT/OT Dept. seems quite good. John Kelly makes himself approachable and gets involved to solve problems. I am very grateful for his help. The facility needs more CNA's and activities on weekends, but overall my Mom seems pretty satisfied.

[review] => So I'm giving an update of our recent experience that my brother and I have had with this facility this Thanksgiving.. Our mother has to have. A guardian to leave the facility because she got confused one day. And started wandering off away from the shopping center where she was being picked up..

So my brother is the closest to the facility, as I live here in California and he called up the facility on Thanksgiving morning to let them know that our mother would be picked up by the Shuttle service, and that the service would be taking her to his house on the other end.

And that my brother would be receiving her right off the shuttle service. My brother called four or five times to get someone and could not reach them and call back again and finally did and let them know. They seem to be constantly understaffed.

What happened next was ludicrous because, the person in charge said that they could not release her, without a guardian and my brother told them very specifically, to remind them that he had full authority as to where she goes and what can be done with her, and that she would be picked up by the service and on the other end that HE Himself would be receiving her physically from the Shuttle and putting her “Physically” on the shuttle service back.
They acknowledged this and said everything was fine and as my brother waited for our mother and 24 guests were waiting over 90 minutes for our mother to show up but to no avail.
So my brother called back to the facility and the idiots there said that they never released her because she did not have a guardian. Why do they have Poor Management and Communication?
My brother called the Administrator of the facility, whom he knows very well and told him in a voice message that while the Administrator was enjoying his Thanksgiving with his family, we surely were not because our mother did not arrive as arranged. Furthermore, on my end, I tried to call my mother and the front desk multiple times yesterday, December 1, 2024, at least 5 times on her direct number, and lo and behold, the phone would not even ring. It would pause for 30 to 60 seconds and then just simply disconnect. I did this five times for each number and I called other numbers, even my brother in Florida, to assure myself that my phone was working properly.
This gets back to the problem that we had in the very beginning; that there is something wrong with the phone system. There are a lot of nice people working there, but they also have a very poor response time, or perhaps it is the phone system itself and I attempt to call my mother directly so I do not bother the front desk. But this is getting very old very quickly and has been persisting for almost two years.
The Administrator has yet to call my brother back yet and address this issue, which apparently he does not seem to care about. If it wasn't for the fact that our mother has made friends there and is happy with the care that she receives, we are not moving her. But we. Really would like to have this addressed as soon as possible and a permanent remedy in place.
We are paying customers and are not getting, Proper service and this needs to be addressed as soon as possible..

[review] => My experience with the Boyton Beach Rehab Center was excellent.... especially with the care and concern from the Administrator, John Kelly. My Mom was at the center for a few short months before she left this earth. No only were the caregivers responsive during her short stay, but also John went way above an beyond to make sure Mom's belongings were cared for until we were able to make arrangements ourselves.
If I were to make a suggestion at all, it would only be that the individuals handling the residents' closes did a better job of making sure the clothes found their way back to their owners.

It was professional but friendly and thorough, with a great deal of information about the facility, activities, and staff. He was delightful and so is his story of his having held several positions at this facility before being promoted to Admissions Coordinator. He's an impressive young man and I was very favorably impressed by the cleanliness and lack of any detectible odor on our tour. Martin is a very valuable asset to his organization.

[review] => I'm extremely disappointed in this facility as I was expected that they would give proper care for my grandpa. He is a very active and strong individual and to hear about the horrific things that happened to him while he was under their care is astounding and I'm SHOCKED. I'm so grateful that my mom, aunt and uncles checked up on him daily. They are the real superhero's in this scenario. His couldn't urinate for 3 days and had extreme discomfort and constantly complained about his degree of pain and they did absolutely nothing about it...They took urine samples but never sent them out to get evaluated. His health went down tremendously within a week staying at this rehab center. No communication and lack of care in this place. I would never recommend this facility to anyone. This place is a joke...

[review] => I would strongly recommend anyone to steer clear of this Rehab Center. They released my dad from the hospital with hopes of getting more Care at the rehab and then going home. Within one week his health deteriorated so much that we were concerned that he would even live. He was neglected many times. Left in the shower room after showering for 15 minutes without a towel and help. Many times he would Buzz for a nurse and they would not come at all or much much later. He had to beg for a shower after a night of diarrhea. His legs and feet swelled up so bad that he had edema and he wound up not being able to urinate and was harshly given quick relief with a catheter which did not work. So he did not urinate for 2 days until we begged for an emergency virtual meeting with the physician's assistant and at least started him on Flomax. If you want to see your family live and thrive you will not go to this place. It needs to be reported. In the end when they heard we were going to call 911 they quickly called 911 to cover their backs.

[review] => DON'T BRING YOUR LOVED ONE TO THIS PLACE. this place is only a nice looking building... the care for patients is worse than any place without care givers. The staff has only two nice male nurses.
The social Worker is very impersonal. The DON asked our father "are you comfortable sir?" and when he answered "no, I'm not", she did nothing at all to assess him and find out what made him uncomfortable. she just said she wasn't talking to anyone and left the room. Our father had a swoll leg under the covers and no one new anything about it.
They told us they were waiting for results on an ultrasound, and happens that they never even had it done...There were orders to arrange for specialists and no one had called to make appointments. They put him in the shower and left him alone and after he was done, he waited 20 minutes for a towel. And he wasn't the only one neglected... as we were standing by the nursing station, other patient' call light were on and we didn't see anyone going in their rooms. Bottom line, we had to call 911 because he started doing worse every day and they didn't do anything to help him in the week he spent there.
all we found was unprofessional people won't listen and use vague excuses to cover for their negligence

I reviewed the orders from hospital. They had 2 doctors he was to see within 3 days. after a week no order for doctors. Finally a nurse at nursing center in a loud voice said nobody does anything here. Get your family out.

[review] => I feel compelled to detail the unprofessional, arrogant, and negligent treatment my 92 year old mother and I received at Boynton Beach Rehabilitation Center (Boynton Beach, Florida) who was admitted to this facility on
December 12, 2022. Less than a week prior, she underwent major surgery (after falling and fracturing her hip) at JFK Hospital (Atlantis, Florida)

While waiting a day and 1/2 for AMR ( American Medical Response)
to arrive at JFK and transport her to BBRehab, she arrived at the facility scared, agitated, and confused. My mother was immediately approached by several staff members who insisted on doing an initial assessment. She was very upset so I suggested they wait a bit - in my mind nothing needed to be done at that exact moment until she calmed down.

Prior to that, the admissions director took it upon herself to give my mother a xanax (not yet prescribed by the physician). This Director is also the same person who insisted I immediately go into her office to tell me that the billing department said they received the "wrong" secondary insurance info for my mother. This director wrote down exactly what I was to tell Medicare and that I needed to call Medicare immediately. I did exactly that only to find out that the director's information was completely wrong - there was no insurance error.

Within an hour of leaving my mother, I was called by the nursing "supervisor" who was working the same day (December 12, 2022) - sometime between 10:00 am and 4 pm) mom was admitted to BBRehab. In a threatening manner she screamed that my mother was BEING DISCHARGED NOW - her exact words. When I asked where to, she said to the JFK ER. I pleaded with her to wait for my husband and I to arrive and speak to the physician but she repeated the same threat and slammed down the phone. She then called a couple more times again screaming "she is being charged NOW!!"

After my mom laid there (at BBRehab) 4 or more hours for AMR to arrive, she was sent back to JFK ER. She arrived alone and confused. I pleaded that someone from BBRehab call me when AMR arrived so I could meet my mother at JFK, but noone called.

I immediately contacted JFK ER and they said "this makes no sense, your mom was dicharged from JFK this morning and after only a few hours she is being readmitted to the same hospital??"

In all my life I have never experienced such
negligent, frightening, and horrendous treatment by "so called" professionals.
What happened with my mother is UNACCEPTABLE.

Our lawyers recommended this place and I am not disappointed. Excellent responsive staff.

[review] => My loved one is a full time permanent resident at this nursing facility. It’s hard to believe this facility can be incompetent at absolutely every level. From their attempt at care to the business office nothing is done right. Food is not edible. The aids are nasty and uncaring. Water requested in the early morning might not be given to the resident until 6pm even after several requests throughout the day. I complain but nothing changes. I can’t speak about any other facility but I can say that this one is bad.

[review] => My mother ended up in this facility for 6 weeks following multiple knee surgeries. New Ownership recently took over and is working hard to turn around the facility which apparently was run poorly by prior owners. The staff here was just amazing, very caring and attentive. Sylvia, the head of administration was just incredible and caring and made sure she was well taken care of by her staff. The nurses, Veluse, Patricia and Bobby and others were very professional and always available whenever she needed help or whenever my family had questions about her status. The PT staff, Brydget and Slim were also great with her rehab and got her strong enough to go home. We ended up referring in another family member who was also in need of rehabilitation. The facility was very clean. While a bit older and not as modern as some others, the staff and care was excellent and I would recommend this facility to anyone.

It’s clean and cleaned often and staff are kind to patients and patient’s family. But most of all when given the opportunity to do so they go above and beyond when it comes to fulfilling the needs of any resident/patient.

[review] => My mother passed away at Boynton Beach Rehabilitation Center on Lawrence Rd. I walked in at 855am and found her dead in her bed. Almost one week later, and still no call from anyone at this facility to give condolonces or explanation of what happened.

All I do know is that my mother was in a shared room, and there was clothing and also a bag with the name of a man on it. His TV was still on when I got there, The bed was dirty/soiled (see pic).

I do see that John Kelly the new administrator who started earlier this year left a 5 star review of this facility. All I can say is that we have used this facility several times over the past 10 years and was usually very pleased with the therapy department for a short term rehabilitation stay. Everything is so different now, so much worse that it is impossible for me to find anything good to say about this facility.

During my mother's short stay, there had been several issues which were not corrected due to lack of supervision. On four occasions the physical therapists brought my mother into therapy around 8am, before she was given her necessary medications. After the second occasion I asked for the supervisor of the department to contact me, yet that never happened. The 3rd time this happened, I contacted John Kelly the Administartor of the facility, and he went in and spoke with the therapy department and came back to let me know that it will not happen again. The very next day, it happened again. There was another occasion when my mother did not receive her antibiotics for UTI for over 48 hours! The aides at this facility did not keep her clean. So I contacted the infectious disease doctor to let him know that the facility still did not have her antibiotics after 48 hours, and he instructed them to send her back to the hospital immediately. At the hospital they decided to keep my mother for the next 5 days just so that she can get her IV antibiotics. I had reported these and several other issues to AHCA and Medicare.

Edit: As you can see, the same unprofessional generic response from senior management below which has been left on other reviews here as well. As of now still no call from John Kelly the Administrator or anyone else with answers as to why I found my mother dead in their facility at 855am, and exactly why she passed away under their care and supervision.

Edit 2: I finally received a call from someone at Boynton Beach Rehab Center, but instead of condolenses or support in our time of grieving the loss of our mother/grandmother/great grandmother, it was a yound lady demanding payment of $150, and said she owes this money even though she is no longer with us.

[review] => BEWARE !!!! Do not let these false reviews by people who work there and family members fool you !! I lost my mother recently who was at Boynton Rehab to regain her strength. Instead they took away her life. They have not called to express their condolences or even explain what happened. They are poorly trained at this facility and can't even get the medications correct. They do not come to the aid of a patient when they are buzzing. We will never know what really happened to my mother . The corruption there starts at the top(John) and works down to the aides. We are heartbroken and there is nothing that can bring our mother back. I hope that this review will help to keep someone elses family member safe.

From Sylvia , and Patti , in admissions, Tasha, business office manager , an Angel sent to us. The entire staff has done a great job making my husband feel welcome and me, as the family member learning to adjust to a very difficult time.
John Kelley, administrator is hands on to anything he can help you with.
Maitenance, Matte was great putting up the TV in a timely fashion. So nice.
Michelle, the nurse has been knowledgeable and helpful with Jareds care.
Norma is the best CNA I have ever known, patients love her.
No one wants to be in a facility, but when the entire staff works together as a team it makes life easier for all.
I thank you all for Jareds care you are giving him. Thank you for your patience..
Daniel, the kitchen manager shows care , and love , to his food to make the patients happy.
I highly recommend Boynton Beach Rehab.

There is only one way to pass bacteria and it on the gloves that they wear all day never changing between patients. They have destroyed our lives with a bacteria that has no antibiotics to kill it. Over three years at home and it comes back no matter what has been given. Filthy under a microscope, filthy. It looks clean but it's horrid. Sepsis every couple months and not once did they call 911. I would rate it zero if possible.

When we were moving him out, we found dead cockroaches, a urine soaked shirt and shorts stuffed in a drawer, and spoiled food with flies in the room near food. The nurses were overworked but were very nice. For the most part, the CNA's were lazy and did not respond to calls. My loved one is paralyzed and cannot help himself or get out of bed for help. We had to ask for a standard sized hospital bed ( he is a big man) instead of the small beds provided. I do not recommend this facility

[review] => After my stroke I went here for rehab - I was treated very badly. I didn’t have my speech back yet but my head was pushed up against the railings while bathing me - leaving me with bruising and bumps above my eye. My daughter has pictures. I couldn’t speak yet and when they would wake me at 5am to wash me up - I would be woken from a sound sleep abruptly and treated very unkind. I was spoken to cruelly at times (as it took me a little longer to understand) and treated like I was nothing - when I finally began to speak - it was only tiny amounts and I slowly began to tell my kids that I was not being treated nicely - through my tears and my small words - my children started staying with me every minute until they could get me out of there. I will never forget how I was treated - I am a massive stroke survivor - my days are still not easy but how I was treated here will never fully leave me

[review] => I was admitted to Boynton Beach Rehabilitation Center following emergency back surgery for an epidural abscess and life-threatening infection. I was on a regimen of IV antibiotics 3 times a day through a PICC line which had been inserted during my hospital stay.

The first four days at Boynton Beach Rehab were pure misery. They did not even order my medications until day 2. They had some IV antibiotic on hand, but I still ended up missing 6 doses due to lack of supply. Their pharmacy is in Miami, and it takes a few days to get medications straightened out. It is unacceptable to miss doses of essential medications due to an inefficient pharmacy arrangement. My parents attempted to reach the administrator many times, but calls were never returned. Then we found out that the administrator was no longer working there. The new administrator did return calls, but didn't seem to fully understand my situation.

It is a good thing that I am alert and very tuned into my needs. More often than not, I had to remind the nurses when it was time for my IV antibiotics. When the IV was finished I often had to wait over an hour (2 hours one time) to be disconnected from the IV. At times I had to remind the nurse to flush the PICC line before and after delivery of the antibiotics. Some nurses didn't even wear gloves or use an alcohol wipe to clean off the part of the line that connected to the PICC line. It is a miracle I didn't come down with an awful infection in the PICC line.

Patients are only given showers twice a week. I am an active 59-year-old and am used to showering daily. With my mom's help, I sponge bathed on the other days. My hair was beyond disgusting by the time I got my next shower. They limit the showers because they are very understaffed. Patients suffer due to understaffing.

The food was inedible. Fortunately my parents brought me food for lunch and dinner. The meals were unappetizing in both appearance and taste.

On a positive note, the therapy was very good. The therapists are friendly and encouraging. After 2 weeks, I received no more therapy since I had achieved their goals.

I was supposed to be discharged to home health after 2 weeks. I ended up staying 17 days longer without any therapy because social services was unable to coordinate the home care. I am not yet on Medicare, and many agencies do not accept commercial insurance. I was to have physical and occupational therapy, a regular nurse and a nurse to maintain the PICC and train my mom and me how to administer the antibiotics 3 times a day. Social services had us on a 17-day rollercoaster of thinking I would go home the next day. Then more paperwork or another signature was needed. One provider turned my case down because of lack of cooperation from Boynton Beach Rehab. The agency would request information, and inadequate information would be sent from Boynton Beach Rehab. This vicious cycle went on for the entire 17 days. So here I am a healthy alert patient stuck in a nursing home for 17 extra days. Some of this is due to the insurance company which finally admitted it is cheaper to keep me warehoused in a nursing home rather than sending a nurse to my home.

Finally my infectious disease doctor (not from Boynton Beach Rehab) made
arrangements for me to receive the infusions at Bethesda East as an outpatient in the infusion center. At this point in my care, the antibiotic was able to be changed to something once a day. Once I had the appointment at the hospital, I packed up and left after the staff on the unit was unsure of exactly what to do when discharging a patient.

Some of the nurses and nursing assistants were rude to both me and my parents. We were all concerned that I was not receiving appropriate care, and they growled at us in a demeaning way. It is not the patient's or family's fault they are inadequately staffed.

When considering a rehabilitation facility for a loved one, I would steer clear of this inefficient place

[review] => If you treasure your loved one’s life, please seek help elsewhere. This facility does not deserve one star, There are staff problems, internal problems, and an overall consensus that patients needs, wants and desires are secondary to the CNA’s break time, lunch time, and free time. This facility needs to be shut down, reorganized, and reopened with caring, educated, and mindful staff that understands the patients needs come first. Please look elsewhere, for this center is NOT the place it advertises to be. Continual mistakes, ignorance, and lack of common sense abound. Care, concern, and common decency are non existent.

[review] => We couldn't be happier or feel more secure with Boynton Beach Rehab. It wasn't an easy decision for our family to place our 97 year old Dad in a nursing home. He has dementia and it's not safe for him to be at home. I wish with all my heart that we could have flown him to be with us during this time, but it's not feasible or safe for him to travel such a long distance at his age and with his ailments. His longtime doctor has always suggested BBR for rehab stints in past years. This time his doctor suggested BBR for his long term care since he could not be home safely. I visited the facility for the first time last week and I was immediately put at ease by the incredibly friendly, warm and attentive staff who greeted my husband and I at every turn to see if they could help us, direct us and answer any questions for us. Nicki, Donna, Jessica, Norma, Sharon, Savannah, Dee and Martin are the people we have connected with over the phone, through emails and in person. It's not the fanciest place and I mean that in the nicest way. I can see why Dad's doctor highly recommends BBR. He says they have the best staff and he's right. We were there over a two day period and so many came to greet us and introduce themselves to us, others came over to Dad to say hello and check in and to tell us how much they like him. We left knowing that Dad was in good hands and was receiving good care and loving attention. And we know he's safe. He likes to wander and they knew this from the get go. They keep an eye on him and we rest easy knowing these wonderful angels are caring for our Dad. Gigi, the beautician at the facility cut his hair while we were there and he feels great about it. There are doctors who come to the facility for foot , eye and dental exams. We're sad about a lot, but we're also incredibly grateful for so much. Knowing our Dad is in such caring and capable hands makes us worry a little less about him. The staff ensures that he'll go to the dining rooms for meals and take part in some activities. He has a walker and a wheelchair and he walks or rolls around for exercise. He's had a rough few months of medical issues and hospitalizations but I sensed a newfound peacefulness about him. He's not as agitated or as nervous as he was when we last saw him in September. He loves to be around people and I suspect being with others after 20 months of isolation is doing wonders for him. Though he comes in and out of his confusion, he does know us when he sees us and he does remember long ago memories. And he loves to sing. He sang to us when we visited him. So while he isn't with us as we had hoped, we are grateful to BBR and their wonderful wonderful staff for making him feel so welcome and for taking such great great care of him.

[review] => Unfortunately I had my husband in their "care", or lack of care, for two weeks. He was admitted into the "skilled nursing" unit. When the ambulance brought him in, they left him on the bed, without anyone coming in, for two hours until we found the nursing supervisor. During his stay there, there was no one to attend to him. He was blind, hearing impaired, and needed someone to feed him. I went to the administrator twice with complaints and was told everything would be taken care of. Finally, the day before his death, he was put into hospice in Delray. i would suggest anyone needing a facility lake Boynton Rehab stay as far away as you can. It is despicable that it hasent been investigated by the Hospital committee.

[review] => My father was a patient there for two weeks before we moved him to hospice. During that time he fell at least twice and injured himself. He was totally blind and very hard of hearing nobody ever cut his food or spoke to him, they lost his hearing aids and do not put his teeth in it. We filed a complaint with DCF but the cure did not improve despite the Administrator providing lipservice to my mom and I sure her that things would be better. They are grossly understaffed and have one LPN working on a unit that has three Corredor’s. My father died the day after we moved him to hospice and he did not have a terminal illness. I would not send anybody to this facility.

Didn't give my father an IV for 4 days because they said they ran out of IV stands... seriously? Food horrible. Never returned calls. left multiple message for the administrator of the facility (Nicki) and Director of Nursing (Jackie) and not once got a return call. Moved my father out after only 2 weeks. It is amazing the care he is currently getting. It is like we went up from a 1 star hotel to 5 start.

[review] => My husband is in Boynton Beach Rehab. The staff is very slow to act. He doesn't even touch his food. He barely eats anything, and he says the food is horrible. I've seen a couple meals, and they're not appetizing to look at. He also needs help with cutting because he can eat independently, but he can't use a knife. He can't properly cut his food, which they didn't do. For the staff, nobody knows anything. When you ask a question, they'll say, "Go ask that lady," and when you ask her, she says, "I don't know, go ask that lady." You're running around in circles, and you don't get answers. They wash the floors, and they clean it. It seems to be clean to me, and I never saw a creature running on the floor or anything like that. They sit there constantly with their waxing machines, and they're always on top of the cleanliness. For the activities, they have a big schedule on the wall. They list a lot of activities, but I never saw anything going on, but I didn't walk around the whole place. It's not that he is being ignored, but everything is done slowly. If he needs a change, it will take more than an hour for someone to change his underwear. You press the nurse's call button, it takes them 15-20 minutes to show up, and sometimes you have to press it again. The staff is very slow to move, but once they do move, they're OK. If he needs help to get in and out of bed, it takes a while to get the staff to do it, but once they come in, they're fine. Therapy is good, they worked with him, and improved him from when he got in there, so I would say the therapy is pretty good.

Every person I have spoken to thus far has been caring and sweet
I am very happy with how careful they are being with screening visitors for covid. It’s hard not being able to see your loved ones as frequently, but, it feels better to know they are safe!!
They are doing their best to get my Dad admitted, for, he needs the care as well. I look forward to continuing to write positive reviews and will keep everyone abreast of our experiences moving forward🙏

[review] => Our family had several negative experiences. They had no floor or bed protection for persons who were a fall risk. My mom was discharged from the hospital for rehabilitation from a broken hip. My mom fell out of bed and was back in the hospital. The EMT reported staff did not know how long she was on the floor. The hallways smelled of urine on some afternoons and they lost most of her clothes she was admitted with.
The recreation staff were very nice and helpful.

[review] => I must say at the beginning of this process I was concerned about sending my family member to Boynton Beach rehab because of some of the reviews I read but fortunately my experience with them has been extremely positive. They try very hard to meet the needs not only of the patient but of the family. They realize that this situation effects everyone . Are there glitches of course but the whole staff works with the family to solve the problems and insure that any situation that arises is delt with promptly. The staff from supervisors to nurses and aides work cohesively to keep the patients safe and well cared for. I strongly recommend Boynton Beach Rehab to anyone who needs nursing homecare.

[review] => I am the daughter of one of the residents at Boynton Beach Rehab Center, on Thurs January 21, we were given the sad news that my dad had been infected and tested positive for Covid19 at the facility. My first reaction was to call the facility and get more information regarding my dad’s condition, I left repeated messages and called several times and their excuses were that the nursing staff were at meetings and couldn’t take the phone call. As of today, no one from the staff has returned my calls. My sister was at the facility on Friday Jan 22, and asked to speak to his nurse, and waited more than an hour and no one came to give her an update on my dad’s current condition, she left without speaking with anyone. It’s frustrating to our family to know the type of care they are providing. In addition, family members are restricted from visiting and a covid test is required to be performed at the facility but yet current residents are getting infected by the staff members. I’m concerned with the type of care is been provided.

The therapy group has been very attentive, flexible and has strongly advocated on his behalf as different types of questions and complications arose. We have received support from the administrator and the members of the nursing staff that have gotten to know my dad and our family members as well. It is unfortunate that some of his needs are out of their control, but I have seen the staff at Boynton Beach Rehab reach out to those external parties and lean on them to come through. We see the value in having him at a smaller facility where there can be a more personal touch. One facility highlight is their lovely atrium which provides a nice change of scenery and sunshine.

STOP CALLING... they insist... one would imagine that they would make a note after my in person appearance after the death, and the several phone calls after... these people are horrible. They allow their staff to choose who they will attend and who they won't , the staff will tell their supervisors "you know I don't go in there" and it is tolerated... there is no communication and no consistency in checking basic vitals... [review] => Husband has only been there for three days and have had nothing but issues with the night shift staff. 4 stars for the morning staff caring, attentive, respectful with the exception of one doctor who was dismissive. Night shift 1 star first night took 45 minutes for an aid to come to the room to only be told I'll get the nurse which took another 45 minutes for the nurse to come in and had no resolution for the problem ( needed straps for his breatheing mask that was delivered at 930 pm) it is now 3 in the morning husband has not slept all day, mind you he has a serious heart condition high blood pressure and cancer and needs this machine to sleep, not one medical staff member suggested oxygen, my husband finally requested oxygen and fell asleep around 5 am. Come 8 am morning staff wakes him and demands he get out of bed and bathe. Second day went pretty well cause he slept all day for lack of sleep the previous day, third day day staff excellent took husband to dyalisis which is only down the road from rehab cost to have him transported from rehab to his treatment 65 dollars ( unreal) thankfully I live close by so I am able to take him, upon returning to rehab center (815 pm) the night nurse or whoever is in charge had changed his room ( has an infection) and put him in a room with another patient who has an infection why I ask would anyone want to risk both patients reinfecting each other. So now husband calls for nurse an aid comes in who doesnt understand what my husband is saying and husband doesn't understand her so we are both stressed at this point, husband is tired and weak from his treatment and just wants to sleep. Will keep posting on a daily basis so everyone is aware of what really goes on at this rehab facility. I know your all going to think why not switch facilities husband is on medicare we are not well off to afford a better place plus it's close to where we live so I can be there in 2 minutes.
Continuation- 4th day Saturday evening was given medication for his phosphate which must be taken with every meal (4 each meal) was only given two and that was after he had already eaten so completely defeated the purpose of taking the meds also come to find out he has not been given his heperin shot on a daily basis to prevent blood clots which he needs in order to clear so he may continue with dyalisis, at this point I'm not blaming the nurse as she clearly showed me they have no record of him needing this he also is suppose to get and antibiotic after his dyalisis treatment ( for mercer) which never happened so now where on day 5 with no antibiotic.
Well I could go on but my husband and I have decided to sign him out and be taken care of at home, so if you want your loved one to die sooner then later then this place is for you.funny I see a response from the facility but only for a good review.

Must say since my husband's return to this facility there has been some major changes made for the good. Staff is more compassioninate, caring and the office staff have gone above and beyond not only for my husband but for me also as caring for a sick loved one can be very overwhelming. My rating for this facility is now a 4.

[review] => DO NOT TAKE YOUR LOVED ONE HERE. Is there an option for no stars? Currently writing this from my dad's HOSPITAL room as I removed him from Boynton Beach rehab this morning. The doctor is a joke, the nursing staff is neglectful & under trained. There was one LPN, Kevin that took care of my dad & deserves positive recognition in this otherwise negative review. My dad spent one week there & now has pneumonia in both lungs, is septic, has skin irritation from a leaking colostomy bag they neglected to address, and his kidneys are barely functioning. I will be reporting them to every governing agency I can. They should be shut down & the nursing staff & administrator should be arrested for elder neglect. It's absolutely disgusting the way my father was treated while he was there.

                            [review] => Danger stay away!!!

My dad spent over 3 months there.
He caught MRSA infection at the rehab
Physical Therapist broke his hearing aid.
His false teeth disappeared.

Worst of all he was getting angry and disoriented. He is not like that. I asked them to check for septic from UTI urinary track infection. That's the first thing that should come to mind with these symptoms. Common knowledge. They should know better. The nurse told me she will go to his room and check it. Instead they want to recommend a psychiatrist. Weeks later 3 days after he got released he got hospitalized for severe UTI. He spent the first 3 days in ICU very sick and suffering. Good thing I was able to get the VA doctor to him. He said dad should go right to the hospital. I love the VA

Trying to get someone on the phone can be very hard. Many days and endless phone calls. When you try to get someone on the phone you can spend the whole day on it. First the phone rings for a long time and then it brings you back to reception. This could go on all day. Sometimes you get lucky and it goes through to someone. Most times you leave messages no one gets back to you. A lot of the staff don't last very long. The VA used to use them then stopped. I bet that's because they are terrible.

They never ordered his prescriptions for eye drops. We asked many times. So we bought it and brought it. Then it was thrown out by house keeping.

The pain doctor wanted to lower his opioid pain patch cause of his age. He said to lower it from. 50 to a 37 for 3 weeks then go to 25. You have to gradually get off the addictive drug. I spoke to the rehab pain doctor and he said he agrees and will do it. They did not do it. They went right down to 25 strength even after My sister reminded them again.

After 2 months there I have to remind the kitchen he has diabetes don't give him sugar. They give him multiple foods with sugar and carbs. I call them they say they will take care of it. 2 days later sugar in his meals again. I called them about this for 3 days till someone answered the phone.

They said in the beginning they would set up a meeting about my fathers condition and healing process. This never happened. We asked many times. My sister lives in Boynton and is available.

He was checking out on a Saturday. I spoke to them on Monday and asked them to have the release form ready and 3 prescriptions. Tried to call back on Wednesday to make sure they got it right. There phones were not working all day. Thursday the phones still out of order. I found an email form on there website for potential new customers to get in touch. I wrote and asked them to please help me. Got a call on Friday. They said the girl that took the release info doesn't work there anymore. I went over everything and they said my dad will be ready for release 10 o'clock on Saturday. My sister gets there on Saturday and no release form or prescriptions. No one knows what's going on. My sister had to sign papers to be able to take him out. No way she was leaving him in this rehab.

I have nothing good to say about Boynton Rehab

[review] => If you care about your loved one, DON'T put them in this place! After 5 days of horrible treatment, I was fortunate enough to find the social worker who helped me navigate a way to get my wife out and home. We met with the dietician who seemed interested in providing proper foods for a diabetic, but the kitchen never followed through. The nursing staff allowed her to sit in shit for 45 minutes and only came to assist because I begged them. The air conditioning never seemed to work properly. They even let a lady who fell out of bed stay on the floor for almost 20 minutes before attending to her. Even after I spoke to the unit manager, head nurse, etc. they all tested me and there was no change. Meds came late, insulin given incorrectly, and truly no genuine concern for patient safety or well being. DON'T go near this facility. I have notified the authorities and they promised to do an investigation.

The staff always seems to assist her anytime I ask for anything, and they help with medication. They have private and semi-private rooms which all seemed to be well taken care of. They're actually doing some upgrades in the facility. It's very clean and there's no odor. There are always activities and I actually see the patients doing their own activities, like going to the activities room to play card games and dominos. They have movies and popcorn, and entertainment that comes in.

[review] => This pace is not very caring. My ex wife was here for less than one week, my daughter had me get her out and we had to rehab her at home. Shes only 50 and was in hospital for a month and needed physical and occupational therapy. The therapist were good and the place appeared clean but the staff gave poor care during the day. She sat in soiled diapers for long periods of time when my kids were not there watching over her. when she would call for help because she needed help to the bathroom or have her diaper changed she would wait for hours. During the day they brought food in to her and she asked to have her diaper changed the nurse told her to eat her food then they would come back later to change it. She told the nurse you going to actually make me wait till after breakfast before you will help me and the nurse replied Oh youlll be fine, iLl come back later. My daughter came by later in the day and asked one of the nurses that checked in on her and asked her how would you like it if you mother was in yhis situation and people treated her in this matter . Her response was we dont keep our family members in places like this, we take care of them ourselves. No respect obviously. Big cultural difference.

[review] => Nothing is ever perfect in these places. For every one good worker there are 10 behind who don't really care. Most of the weekly day staff are wonderful, caring and are very helpful. The problems come on the weekends when supervisors/managers are not there to oversee what is going on.so then staff is not really doing their jobs. I have addressed this issue with them and filed a grievance with them. At times it seems as they are working on all the bad things, but then there are times when nothing is being done. If You call you don't always get the person you are looking to speak with to advocate for your loved one. They are always in a meeting, don't answer the phone or it goes into voice mail and you do not get a call back. This happens at all nursing homes even the most expensive ones. It is an education in itself. You really have to pick and chose your battles with them. Ask yourself, is your loved one being fed, being cleaned, being looked after? No one person in there is going to get 100% unless you hire your own personal private CNA to pick up everything that the hired help is not doing. Good luck!

Upon arriving we asked to use a wheel chair for my mother to get to her nieces room which was located in the back of the facility. We were told we could not use a wheelchair. She walked with her cane to visit her niece who is named after her. We passed numerous wheel chairs walking through this facility, but she was determined to see her. This gave me the feeling this place was very unfriendly to family which is an important part of a patients recovery.

I rang for a nurse for my mother in law and TWENTY minutes later one ambled in looking annoyed at being bothered. That happened quite a few times. The food is disgusting, what kind of dinner is a congealed blob of mac and cheese and a dry piece of bread??? This is a rehab, not a jail. My mother in law developed a bed sore after being there. She had never had one before. I could go on and on..

[review] => My mom was there for rehab after having pneumonia and "graduating" from a long term acute care facility where she was for several months. She was at Boynton to get strength back to begin to walk again. At admission, she could not get out of bed, had just gotten off a ventilator, had many medications that needed to be administered, was a heavy woman, and was anxious. She needed caring folks around her and skilled, competent nursing. Unfortunately, that is not what received. Overall, nursing staff is not concerned with patients at all, though there are a coveted few that do, and you feel elated when they are there. They particularly despise heavy people, as it requires 2 people to get one out of bed into a wheel chair, and the rehab is really not "set up" to do this....they must borrow people to help, and the second person is almost always annoyed. To ensure her safety, it required us as the family to have someone there until bed time, and then we ultimately hired a private nurse to watch over mom during the night.. Even so, danger was always present. We had to watch for medication mix ups or negligence at all times, and still encountered that while we were present. In addition, though in the end, physical/occupational/speech therapists did wonders for mom....she amazingly walked out of there with a walker...the first PT/OT team was just bad, threatening my dad every day to "release" her as she was not working hard enough. Turns out she was on the wrong meds, which caused her to be woozy and exhausted. We told PT/OT that this was happening, but, like the nursing staff, they did not seem to care. but Once we got mom on the right meds, which was a feat due to the doctor they contract with actually "firing" us because we asked him too many questions during my mom's medication debacle (not kidding....though we then got a physicians assistant from the other contracted doctor who saw her and was easier to deal with....never saw doctor though) and we replaced the derisive PT/OT team, she began an amazing recovery! Her new PT/OT team had faith in her and she rallied. She as like herself again! Our family got used to having to be there every day...my sister moved from the northeast to be there, and being there allowed her to bing several issues to the administrations attention, including a blood clot in moms arm which went undetected by the staff. In the end, Mom made a wonderful recovery, and is home and walking stairs now, after a full 6 months of being in hospital and rehab. Do I think Boynton is a good rehab? I gave it a 3. If you are protecting your loved one 24/7 and not trusting the nursing staff (and be prepared to constantly manage them/argue with them, as they don't respect families either) If you create a personal relationship with the director and the head nurse (I suggest a meeting and then constant follow up), If you make sure to get the "right" physical therapy team that is caring and non-judgmental (which means creating a relationship with the head of the department...she is very nice) If you get the contracted doctor with the Physicians Assistant who will be accessible to you and not the other who will "fire you" for trying to contact him, If you triple check on everything, from transportation to outside doctors appointments to medication to appropriate food....well then, yes...your loved one can recover at Boyton. But boy, it's an overwhelming burden on the family, as you become the Advocate/Manager/Diagnostician/Protector of your loved one. Is this unique? Probably not. I have been told that Boyton is considered one of the better rehabs in the area by outside nurses, which suggests a sad state of affairs in South Florida. But, if you do all I recommend, your loved may recover., like mine did.Without constant vigilence, however, they may not make it out.

[review] => Great rehabilitation folks, impersonal and rude nursing staff. My mother was admitted after hip surgery and I was able to visit twice a day. She enjoyed the rehab therapy but was having trouble getting attention from the nursing staff regarding her medications and routine care. The morning of her fourth day she said she didn't feel well. By the time I returned in the evening, she was drenched in sweat and incoherent and I was told she had been that way for several hours. The nursing staff were unconcerned and irritated with me for being angry and insisting that the doctor was called. They said it was probably a reaction to something she ate at lunch and they wanted to wait until the doc came in the next day. I forced them to call an ambulance so they ordered one on a non-emergency basis then they all disappeared and didn't move from their chairs at the nursing station again, even when the ambulance arrived over 30 minutes later. By the time we got Mom to the hospital, her blood pressure had bottomed out and she was in renal failure. She was in the hospital for a week with a horrible gastric infection. Nobody from Boynton Rehab has ever bothered to contact her or the family. She did not return there when she was released from the hospital, of course. We're extremely happy with the rehab center she's at now.

There is lack of concern for patient satisfaction and a high emphasize on staff work load and liability issues. There were also 2 mistakes when staff called a non-family to discuss the patient's plan of care which is a Hippa violation, lack of concern or appropriate bathing for the member's hygiene, lack of concern for ADL access for each patient or related frustration, and untimely response when a patient needed help. In one extreme situation, six weeks of dirty clothing were not laundered and underwear with fetus was thrown in the back of an closet. Find another facility since Florida is loaded with rehab agencies.

[review] => The care here is marginal at best. Everyone moves at a glacial pace, and gives you attitude. If you can find someplace else, by all means do so. On a positive note, the food is decent.

[review] => The people are very friendly at Boynton Beach Rehab Center. The place is very clean, and they have good food. My mom-in-law's room is like the size of a master bedroom, and she has her own bathroom. There is room enough for a queen size bed, dresser, and a television.

[review] => My aunt is in Boynton Beach, and it is better in the beginning. The place is clean. Some of the staff is good, and some are not. They aren’t really that attentive; when she first came they were more so, and now that she is long term, she is not getting the attention. They had a very good rehab part, and they were very good with her there.

[review] => The gym and the therapist at Boynton Beach Rehab gym are outstanding. Without them, I don’t even know where she'd be today. When she arrived there, she could not even stand on her own. They had to use that special equipment to stand her up and she walks on her own now. The nursing staff are OK. The office staff could be more tactful, like when they are trying to collect money and stuff like that. The facility is always clean. I would recommend this facility especially for the rehab part.

[review] => My grandmother has been in LTC now for a while, and every time I have visited (from out-of-state), the staff has recognized me as her relative, greeted me and been very warm to me and my family. My grandmother's mental as well as physical needs are very well met in this location, and it is one of the most beautiful rehab/LTC's I've ever seen. My grandmother was in another location prior to this, and even though there were fewer patients in that location, this location is better in terms of staffing (number and quality), food (the food is delicious!), and the rehab specialists are top-notch. I feel so relieved to know that my grandmother is receiving such wonderful care!

All the Staff at Boynton Beach Rehab have been absolutely amazing; from Dr. Attella & Dr. Gorch; Dr. Attellas Nurse (Nickie); Admission (Mary-who helped me SO very much with navigating my moms health and creating the most smooth transition from the Hospital to this wonderful Rehab; she is an absolute gem; (Martin) super organized, helpful and friendly; John (the Director) is phenemenoal. To all the Physical Therapists, Occupational Therapists, Nurses, CNAs & Nursing Students have all been so incredibly friendly and helpful in my moms recovery. Boynton Beach Rehab is extremely well - kept and clean, serves healthy, tasty and balanced meals.
